Any individual or organization, whether they are HAI members or not, may submit nominations, or be nominated for one or more of the nine categories listed in the Call For Nominations Booklet.

To submit a nomination, complete the online form below, or print out and complete the Nomination Form found in the Call For Nominations Booklet. Please give concise details that support your nomination. Supportive materials aid consideration in the selection process. These items may include awards, training certifications, letters of recommendation, pictures, newspaper articles, and magazine articles that relate to your nomination. Please send no more than three items. These items must be suitable for photocopying. Please do not send videotapes and non-paper items prior to selection. All materials submitted for nomination become property of HAI and will not be returned. HAI requires that nominees complete an Acknowledgement Form, confirming that they know they are being nominated and, if selected, that they would be willing to receive the award in person at the “Salute to Excellence” banquet on February 22, 2010 in Houston, Texas, or will designate a representative to do so on their behalf. The nominee must also provide biographical material, including photos, to HAI.
