Rotor & Wing - The U.S. National Transportation Safety Board has published a total of nine new recommendations following the investigation of an August 2011 Eurocopter AS350 B2 crash in Mosby, Mo. Four people died in the accident. NTSB determined that four factors contributed to the crash — training; fatigue; pilot being distracted, including sending personal text messages; taking off with less fuel than needed.

The FAA proposes to adopt a new airworthiness directive for Robinson Model R22, R22 Alpha, R22 Beta, and R22 Mariner helicopters with certain fuel shut-off valves installed. This proposed AD would require replacing the fuel shut-off valve with a newer design fuel shut-off valve.

Honolulu, Hawaii (HawaiiNewsNow) - A routine afternoon in Downtown Honolulu turned into one of fright and alarm just before 3:20 p.m. on May 8, when a small helicopter made a hard landing onto a city street, skidded nearly 50 feet, then hit a parked car.
